Maybe I'm missing the point, but it seems to be missing most of the features needed to be useful.
I cannot think of a scenario when I would want the user to type in a description and amount for the charge.
On the positive side, it works. The code looks clean and well documented. This might be useful as a starting point for someone who wants to program their own plugin.

Great plugin! It works flawlessly and is a lot easier to figure out and configure than alternatives that I had previously tried.
